CORTEZ SEAT BASE RETARDANT TANK CLEANOUT

This solicitation amendment, 004, is to extend the responses due date, extend out the POP start date, correct the POP end date on the SOO and answer additional questions that were received. Please see the attached question and answer document. The new questions and answers are in red. Please be sure to sign and include any solicitation amendments in your quote package.

Cleanout of 8000-gallon capacity metal tank of fire-retardant liquid concentrate remainder/sludge. Estimating 500-1000 gallons of sludge in bottom of tank plus additional water needed for removal/cleanout.

Cleanout of 3 poly tanks (3000-gallon capacity each) with residual mixed fire retardant. Two (2) tanks contain estimated 50 gallons each of dried/solidified mixed retardant, 1 tank contains estimated 50 gallons of sludge.

Basic Contracting Requirements:
1. Active Registration: www.sam.gov/SAM
2. Compliance with Section 889 clauses.
3. Invoicing and Payment: www.ipp.gov - 30 day prompt pay after delivery with Progress payments allowed for completion of at least one tank.

5. FAR Part 13 - Simplified Acquisition Procedures
6. Small Business Set-Aside for NAICS 562211 - Hazardous Waste Treatment and Disposal- Size Standard: $47M
7. Type of Contract: Firm Fixed Price.
8. Award of this contract will be on a Low Price Technically Acceptable (LPTA) basis looking at the following factors.

a. Technical Response: Include a description of your technical approach, safety plan, certificates and qualifications, expected timeframe to complete the project, equipment, supervision and personnel. Include a description of experience in performing jobs of similar scope. A proposal will be considered technically acceptable if the vendor shows they have recent and relevant experience with similar types of projects, proper equipment, necessary personnel, certificates and qualifications, resources and equipment and ability to safely and effectively complete the project within the allotted timeframe. Technical factors will be assessed as acceptable or non-acceptable.

b. Price: Among the technical responses assessed as acceptable, the award will be made to the lowest price.

- Please email or call with any questions you have, prior to the closing date, with enough time to research an answer.
- Email your signed quote package to: kspetter@blm.gov, with "140L1724Q0001-Canon City Retardant Tank Cleanout" in the subject line, no later than the closing date. Include your Unique Entity Identifier (UEI) which replaced the DUNS number on your quote.
- You may put your prices on the SF-18 form itself.
- Include enough description/information in order to determine technical acceptability.
- Offers are due November 17, 2023 not later than 17:00 hrs (5:00 PM) Mountain Time (MT).
