Blanket Agreement for Shipping/Packaging Supplies

*THIS RFQ WILL BE EVALUATED BASED ON LOWEST PRICE, TECHNICALLY ACCEPTABLE METHODOLOGY*

This request is for a one-year blanket purchase agreement for shipping supplies and packaging materials - such as boxes, tape, labels, polybags, etc. - on an as-needed basis. Please see the attached Appendix C Item List for a specific list of commonly purchased items.

Please review all applicable documentation and provide a proposal response no later than 10:00 AM CST December 18, 2024.

Any and all questions must be submitted no later than 5:00 PM CST December 15, 2024.

Within your proposal, please include:

1. A signed copy of the ANL-70 RFQ Document
2. A completed copy of the PD-158 Document
3. A completed copy of the Appendix C Item List
